Consent to personal data processing

By submitting any form on the melnychenko.law website, I, as a data subject, in accordance with the Law of Ukraine “On Personal Data Protection”, voluntarily consent to attorney Serhii [Patronymic] Melnychenko (certificate of the right to practise law No. 0000 of 00.00.2014 issued by the Kyiv City Bar Council), practising as Melnychenko Law Office, address: Office 00, 00 Prykladna St, Kyiv (the “Attorney”) processing my personal data on the terms set out below.

• surname and first name (or the name I gave);
• phone number, email address, messenger username;
• country of residence and preferred contact channel;
• information about my situation that I chose to give in the form;
• for media enquiries — the name of the outlet and my position.

• contacting me about my request, consultation booking or media enquiry;
• a preliminary assessment of my situation and preparation for providing legal assistance;
• concluding and performing a contract for legal assistance, if one is concluded.

I agree that the Attorney may collect, record, accumulate, store, update, use, anonymise and destroy my data, with or without automated means.

My data may be processed by the attorney’s assistants and by technical service providers (hosting, email, notification services) only to the extent needed for the stated purpose and in confidence.

The consent is valid from the moment the form is submitted until the purpose of processing is achieved, but for no longer than 12 months if no contract for legal assistance is concluded, or for the periods set by law if a contract is concluded. The consent may be withdrawn earlier.

I have read the rights of a data subject set out in Article 8 of the Law of Ukraine “On Personal Data Protection” and the Attorney’s Privacy Policy, and I understand that information shared with the Attorney is protected by attorney–client privilege.

I may withdraw my consent at any time by writing to office@melnychenko.law or calling +38 (067) 000 00 00. Withdrawal does not affect the lawfulness of processing carried out before it. After withdrawal, my data will be deleted within 10 business days unless the law requires a different retention period.
